Who:    Healthy young people from age 6 months through 24
            Pregnant women
           Adults 25 to 64 who have underlying
                                chronic medical conditions or weakened immune system
           People who live with or care for infants younger than six months of age
           Health care and emergency medical personnel

Parking is available in the school parking lot.
Bring your completed consent form.  Make sure to sign both sides of the form.
Children should have a loose fitting top to allow access to upper arm for flu shot, if necessary.
One shot/dose is required for anyone over the age of 10.
Nine years and younger must receive 2 shots/doses.

If you have any questions about the vaccine or the vaccination clinics, please call 309-925-5511 from 8:00 a.m. to 4:30 p.m.  Please visit the CDC's 2009 H1N1 influenza web site at http://www.cdc.gov/h1n1flu/ and also http://www.cdc.gov/h1n1flu/parents for information especially for parents.  Your child's health care provider also can answer your questions about he 2009 influenza virus.

 

****Please note that Tazewell County Health Department 
could cancel this clinic if the necessary immunizations are not delivered on time.
